Social Services
Our professional Social Services Department plays an important role in helping residents adjust and thrive in our community. In addition, you can count on our staff to serve as advocates for patient and family rights, working to resolve issues concerning illness, placement within the facility and lifestyle changes. You can also turn to us for information on topics such as heart disease, diabetes, cancer, Alzheimer's and other medical educational materials. Additional services include: financial and insurance information, advanced directives information, family support groups, and bereavement support groups.
When patients complete their short-term rehabilitation, our Social Workers coordinate the services needed following their discharge, such as continuing care from a visiting nurse, home health aides, Meals on Wheels, or physical and/or occupational therapies at home, as well as any equipment needed.
